Our 2025 North Mt. Juliet Rd office has moved!!! We are now located in  Heritage Medical Building located at 325 Old Pleasant Grove Rd, 3rd floor, Mt Juliet TN, 37122.


On Saturday's all appointments will be in the Mt Juliet location.



If your child has not been seen in the clinic since November 1, 2025, please go to the link below to set up the new MyChart portal. You will need to have a parent MyChart account set up first and your child can then be linked to your account. Our front desk staff is able to assist with completion of this process.

Self scheduling is now AVAILABLE for those that have MyChart app set up. If your child is over 13 please make sure you complete the proxy form in clinic to be able to CONTINUE WITH PORTAL ACCESS. Our front desk staff are able to help you with this process. Telehealth appts are also done through the MyChart portal access.


 Vaccinations available in office by appointment. Please talk with your child's pediatrician to see if your child would benefit from one of the following vaccines.

 

  • Influenza
  • Covid
  • RSV
